Please read the following guidelines prior to completing any application or membership form.
The first step in the application process is to consult the CSPA Membership Requirements and then select the type of membership you are seeking: CSPA Professional Member or CSPA Student Member.
CSPA Professional Members
1. Becoming a new member
You may apply to become a CSPA Professional Member at any time during a given year, however, applications will only be reviewed once a year during the month of September. Applications must be submitted no later than September 15th each year in order to have it reviewed at that time.
To apply to become a CSPA Professional Member, you must complete all required application forms which include:
Form 1 - General Application Form
Form 2 - Supervised Practice Grid
Form 3 - Supervisor Appraisal Form
Form 3a - Verification of Supervisor Form (If your supervisor is NOT a member of CSPA or is a member but does not have 5 years consulting experience then your supervisor will need to complete this form and send it to the Chair of the Review Committee for pre approval.)
Form 4 - Client Appraisal Form (two clients who will evaluate your consulting work
In addition, you must submit an application fee in the amount of $ 30.00 to Natalie Durand-Bush (see Geneal Application form for address). All forms must be emailed to the Chair of the Review Committee, including a pdf. version of official transcripts. Applicants must also mail a hard copy of any original transcripts.

2) The applicants must send an original hard copy transcript from all academic institutions to the Chair of the Review Committee.
3) The applicants supervisor must send by email the Form 3 - Supervisor Appraisal Form, if necessary Form 3a Verification of Suervisior Form and CV to the Chair of the Review Committee.
4) The Form 4 - Client Appraisal Form must be sent directly from the client to the chair of the Review Committee
Upon submission, your validated application form will automatically be sent to the 3 members of the Review Committee (RC) who will evaluate your application.
2. Renewing your membership
Your CSPA membership expires on December 31st. You will receive a notice to renew your membership prior to membership expiration.
CSPA Student Members
1. Becoming a new member
You may become a CSPA Student Member at any time during a given year by completing the CSPA Student Application. If you have further question, please contact Judy Goss, Chair Membership Committee at judy.goss@cscontario.ca. A fee of $40 Canadian is required with the application (see application form for payment information).Upon confirmation of payment, your membership will be activated.
2. Renewing your membership
Your CSPA Student Membership expires on December 31st each year. You will receive a notice to renew your membership prior to membership expiration.
